Objective: To observe the therapeutic effects of primary thrombocytopenic purpura treated by TCM differentiation and treatment.

Methods: 20 cases of primary thrombocytopenic purpura were treated based on TCM differentiation. Among them, 7 cases were deficiency of liver-Yin and kidney-Yin type, 3 cases deficiency of both heart and spleen, 8 cases deficiency of spleen and kidney, 2 cases domination of heat and blood stasis.

The treatment should be invigorating Qi and nourishing Yin, invigorating Qi and nourishing the blood to treat originally, and promoting blood circulation to remove blood stasis, clearing away heat and toxic material to treat the superficiality:

(l) For deficiency of liver-Yin and kidney-Yin type, Modified Zhi Bai Di Huang Pill should be used, which contains l0g of phellodendron bark, l0g of Anemarrhena Asphodeloides, 20g of prepared rehmannia root, stir-fried tortoise plastron, l5g of Chinese yam, l0g of dogwood fruit, l0g of Alisma Orientale, l0g of Poria, l0g of moutan bark, l0g of red peony root, l0g of White peony root, l5g of buffalo horn and 6g of powder of Placenta Hominis;

(2) For deficiency of both heart and spleen, Modified Gui Pi Decoction should be chosen, which consists of l0g of dangshen, l5g of pseudostellaria root,10g of baked bighead atractylodes, l8g of fresh astragalus root, l5g of Chinese angelica root, l0g of Prepared liquorice, l5g of poria, 10g of polygala root, l2g of baked wild or spiny jujuba seed, l2g of aucklandia root, 30g of Arillus Longan, 20g of buffalo horn, 10g of red peony root, l5g of white peony root, l0g of donkey-hide gelatin, 6g of Placenta Hominis,

(3) For deficiency of both spleen and kidney, Gui Pi Decoction, Er Zhi Pills and Modified Sheng Mai Drink should be used. The Prescriptions contain 20g of pseudostellaria root, l5g of ophiopogon root, 10g of schisandra fruit, l0g of phellodendron bark, l0g of Anemarrhena Asphodeloides, 20g of prepared rehmannia root, 20g of tortoise plastron, 20g of fresh astragalus root, l0g of baked bighead atractylodes, 10g of red peony root, 10g of white peony root, 25g of buffalo horn, l5g of glossy privet fruit, 15g of eclipta, 6g of powder of Placenta Hominis and donkey-hide gelatin;

(4) For the type of domination of heat and blood stasis, Modified Xi Jiao Di Huang Decoction should be chosen. Results: After 2-3 months of treatment 8 cases, 40%, were obviously effective, 8 cases, 40%, effective, 2 cases, l0%, improved, 2 cases, l0%, ineffective, the total effective rate being 90%. The changes of BPC of 20 cases, BPC 27.l X l09/L±X 22.0X109/L increased gradually to 67.15X109/L±l4.93 X l09/L after giving the medicines. There is significant difference before and after treatment (P<0.0l).
