A clinical gout case

Gout is a kind of disease caused by prolonged purine metabolic disturbance, clinical characterized by hyperuricemia, repeated attack of acute arthritis gouty tophus sedimentation, chronic arthritis with deformity of joints, pathogenic change of renal parenchyma and formation of tophus. Generally speaking, gout can be caused by hyperuricemia due to the increase of uricopoiesis. Mostly this disease may occur at the age of 30-40. Usually the patients have family history of gout. If long-term colchicines are used, the liver function and renal function will be affected.

In TCM, gout pertains to bi-syndrome, externally it can be caused by exogenous pathogenic wind, cold and damp heat. Internally, it can be caused by Qi insufficient, overstrain, or exposure to wind , and preventive Qi fails to protect the body, which give birth to blockage of the meridians and stagnation of Qi and blood. The pathological factors are dampness, phlegm and blood stasis.

A clinical gout case

Lijianguo, male, 44-year old.

Ten-year gout history. Family history.

Symptoms:

Ankle and dorsum of feet pain. Gouty tophus sedimentation on legs and feet. Addicting to alcohol, meat and sea food. He has taken colchicines for a long time. Purple tongue and slight taut pulse.

Lab test:

UA 508.60ümol/L (90.oo- 430.00)

ALT 133.70 IU/L (0.00-40.00)

GGT 85.20 IU/L (0.00-50.00)

Diagnosis:

Dampness blends with blood stasis and blood deficiency.

Prescription:

Chishao 15g   Chuanxiong 15g   Danpi 15g   Weilingxian 15g

Ezhu 10g     Shancigu 10g    Qingpi 10g   Xiakucao 10g

Chuanniuxi 15g   Zhuling 10g   Zexie 10g    Fuling 10g

Decoct the above drugs in water for oral application. (7 dosages)
