                       Clinical experience of Yangbingkui to treat brain tumor

1. Pathogenesis is deficiency of liver and kidney with phlegm-heat
Professor Yang thinks that the location of brain tumor is brain, but it has the relations with liver, kidney and spleen. Deficiency of the liver and kidney is the root of this disease. One is liver-wind can be caused by extreme heat or hyperactivity of yang due to yin deficiency to disturb brain, the other is deficient kidney essence to be attacked by pathogenic factor easily. Dysfunction of liver and spleen will effect the transportation of qi, blood and fluid, which induce dampness and phlegm, accumulated dampness and phlegm generate heat. Heat and wind attack brain to cause brain tumor.
Professor Yang divides pathogenic factors into external and internal factors. External pathogenic factor includes chemical toxicant, poison of environmental pollution and food, etc. while internal pathogenic factor includes phlegm, dampness and blood stasis, etc. these pathological products accumulate to generate phlegm poison (Tandu), damp poison (Shidu) and stasis poison (Yudu).
If the Yang external pathogenic factor attacks Yang constitution, it will cause the heat poison which will consume Yin fluid. If the Yin external pathogenic factor effects the movement of Yang qi, it will cause Yin excess cold in interior firstly, and then accumulating to generate heat poison. For the Yin constitution, asthenic fire can generate poison. All of the pathogenic factors can be mixed together with blood stasis, phlegm and dampness, accumulating to obstruct in brain collaterals for long term, inducing tumor or cancer. So the treatment of using poisons as antidotes is effective clinically.
2. Syndrome of differentiation combining with diseases and symptoms
Professor Yang concludes that treating methods should be made according to syndrome of differentiation. It can be divided into 5-syndrome types in clinic.
(1) Stagnation of phlegm-dampness type: dizziness and headache, nausea, vomiting phlegm and saliva,rale in pharynx, heaviness, poor appetite, light-color and big tongue with white and greasy fur. Ditan Tang (涤痰汤) with Xiakucao Gao(夏枯草膏) are used.
(2) Qi stagnation and blood stasis type: severe headache, aggravating sustainability or paroxysmally, fixed pain, dull complexion, hemiplegia, dry constipation, dull and purplish tongue with petechia, purplish and thick choroids under the tongue bottom. Modifying Naoliu Yin(脑瘤饮加减)is used to activate blood and anti-swelling.
(3) Excessive heat-toxin type: headache, distending feeling in head, vomiting frequently, bitter taste in the mouth, brown urine, dry constipation, red or white dry tongue, wiry and quick pulse. Modifying Longdan Xiegan Tang (龙胆泻肝汤加减)is used to purg fire to eliminate toxin.
(4) Liver-kidney yin deficiency type: dull headache,intermittent onset, tinnitus, dizziness, unclear eyesight, numb limbs, dry constipation, short and brown urine, red tongue, with little fur and thin pulse. Modifying Qiju Dihuang Pill (杞菊地黄丸加减)is used to nourish liver and kidney.
(5) Deficiency of both qi and yin is onset after operation, chemotherapy or radiotherapy, hypodynamia, short breath, spontaneous perspiration, dry mouth, poor appetite, night sweat, dull complexion, light color tongue with thin fur, Modifying Jiawei Sijunzi Tang (加味四君子汤加减). On the base of the above types, Professor Yang put forwards that for severe headache, add insect drug to expel wind and dredge collaterals, such as Scorpion, Lumbricus, Gecko, etc. For severe headache like awl and thorn, add Rhizoma Sparganii , Zedoary Turmeric, Prepared Pangolin, White Peony Root to activate blood and remove stasis. He pays attention to used guiding drugs, for example, for prefrontal headache, add Root of Dahuriae Angelica; for bilateral headache, add Bupleurum; for the pain of occiput, add Kudzu; for the limb numbness, add Milettia Reticulata, Curcuma Longa, Achyranthes Root; for the vomiting patients, add Bristle Inula(decocting wapped medicines), Haematitum (Decocting first ) Orange Peel, Pinellia, etc.
For glioma, add She Liugu ,Herba Hedyotis Diffusae, Rhizoma Arisaematis, Scorpion, Chinese Lobelia, etc. to clear heat and detoxify, anti-cancer. For Meningioma, add Prunella Vulgaris, Zedoary, Laminaria Japonica, Seaweed, Santsigu Tuber, etc. to soften hard lumps and dispel nodes; for metastatic tumors originated from lung, Sun Euphorbia, Santsigu Tuber, Fritillaria, etc. metastatic tumors originated from liver, add Barbed Skullcap, Seeding of Virgate Wermwood, Herba Hedyotis Diffusae, Fructus Polygoni Orientalis, Folk Herbal Medicine; metastatic tumors originated from breast, Trifoliate Orange, Seed of Cowherb, Pangolin, etc. Professor Yang concludes that She Liugu (Dragon Arum) is pungent and bitter in flavor, toxin, can eliminate dampness to reduce phlegm, dispel wind to stop convulsive, dissipate lump and mass, and be effective to treat cancer, usual dose is 10-15g (decocted first 2 hours to reduce the toxin.).
3. Case
Dai, Male, 21-year old.
The patient had the surgery of brain tumor resection for top right after the tumor, pathology after surgery showed primitive neuroectodermal tumor. Radiotherapy and chemotherapy were used. The tumor was recurrent on Aug.10, 2001, operation again, postoperative pathological showed that glioma of ganglion cells(n grade). Cranial MRI reported that Primitive Neuroectodermal Tumors of right top, strengthening points in right basal ganglia, tumor recurrence. First visited on Sep. 04, 2001, the patients showed hypodynamia, headache and dizziness, no diplopia, normal excretion, large tongue with tooth-marks, light reddish tongue with thin fur, thin pulse. Activating blood and removing stasis, softening hard lumps and dispelling nodes. Prescriptions: Lanceolata 15g, Astragalus 15g, Prepared Atractylodes 12g, Dragon Arum 12g (decocted first 2h), Herba Hedyotis Diffusae 12g, 3 Centipedes, Arisacma10g, Prunella Vulgaris 15g, 3 Scorpions, Chinese Lobelia 20g, Zedoary Turmeric 20g, Fritillaria 15g, Rhizomaligusticichuanxiong 15g, 3 Geckos, Coked Medicated Leaven 15g, Fried Malt 15g, Coked Hawthorn 15g, Liuwei Dihuang Pill (wrapped decocted) 12g, Baked Licorice 5g. The patient had cranial CT on Jan. 15, 2002, showed that right posterior limb of internal capsule, part of white matter of temporal lobe and parietal lobe were mild edema. The patient insisted on taking the above prescription, recovering mostly.
Comments: this case is intracranial tumor, Professor Yang thinks that it is caused by deficiency of liver and kidney, with heat-toxin and phlegm inducing deficiency of Yin and Qi. Activating blood and removing stasis was used firstly, and then nourishing liver and kidney, modifying with wind and phlegm-removing herbs, used insect material herbs to dredg collaterals, eliminating sputum softening hard mass herbs were used to guide the herbs upward, supporting vital-qi and eliminating evils,which gained satisfied effect.
